Donation Request Form

We receive many requests for donations, and although we would like to help everyone, logistically, it isn’t possible. When asked to donate to a school or other non-profit, we must carefully consider several factors. To help the process, please fill out the donation request form completely when making a request for a donation from the Columbus Collective clubs. Please ensure a minimum of 45 days in advance if it is needed for a specific date.
